The purpose of this feedback is to assist the coaching staff in developing themselves and the program for the future.
The Email address and name entered must match our registration for the feedback to be reviewed. Feedback submitted using Fake names/emails will be discarded.
Feedback will be confidential and used to assess and evaluate our volunteer coaches as a tool for improvement. No names or personal info are shared with coaches. Please submit 1 evaluation per player playing in our program.
